Tattoo Consent Form
In order for a minor (under 18 y/o) to get a tattoo, you must have a notarized consent form. These can be obtained by picking up a physical form from our shop, or by clicking the link below:
- The consent form can only be filled out in front of the notary, not at home.
- If the minor would like to do several tattoos, list them all on the consent form. The form is valid for 6 months.
- The legal guardian and minor will both need to have a valid state-issued photo ID
- The original birth certificate, or proof of legal guardianship is needed as well. E-mailed, photocopied, or pictures of the birth certificate are not valid forms of ID.
- Accepted forms of valid photo IDs are driver licenses, passports or state IDs.
- All forms of ID used with the consent form must be brought to the tattoo appointment.
- The legal guardian must be present for the entire tattoo appointment.
- Florida law does not allow anyone under 16 y/o to be tattooed under any circumstances.
We accept tattoo walk-ins every day depending on our availability, but we suggest making an appointment! By paying a deposit to set an appointment for a tattoo, you confirm that you have read and agreed with the policies for a minor to get tattooed.
Although we do accept walk ins, we do suggest calling ahead before coming to double check our availability as we take walk ins on a first come first serve basis. You are also welcome to make an appointment to reserve a specific date and time. By paying a deposit to set an appointment for a piercing, you confirm that you have read and agreed with the policies for a minor to get pierced.
